Accessing Our Patient Portal
Prior to your first visit, you will need to have an invitation from our office to access our patient portal to complete registration information. If you have not yet contacted our office, please call our office first to schedule an appointment prior to attempting to access the portal.
For existing patients of the practice, when you have a new problem, call the office for an appointment and the staff will direct you to the portal to update your information.
If you arrive for a scheduled new patient or re evaluation/new problem appointment without your portal registration completed, we will likely have to reschedule your appointment.
If you cannot access the portal, please contact our office for assistance. If needed, it is possible to fill out paper registration forms in office. Call our staff and they will arrange to have you arrive 20 -30 minutes prior to your scheduled appointment time to complete the forms.
